  • Health Management Strategies for Recovery

    Integrating primary care and mental health care in one setting helps people with serious mental illness access timely, responsive and quality care to identify, respond to and prevent chronic and life-threatening disease.

    Bridges, through its’ CommuniCare partnership and in collaboration with Cornell Scott-Hill Health Corporation, offers on-site primary care and wellness services. The Cornell Scott-Hill Health Center provides licensed primary care at Bridges one day per week.

    While we are in the early stages of this initiative that is becoming the model of care throughout the nation, we are confident that our efforts are saving lives.

  • Health Management Strategies for Recovery provides primary care screenings and treatment as well as:

  • *Access to a ten-session wellness curriculum
    *Individual and group health and wellness education
    *Wellness plans
    *Community health lectures
    *Family wellness education
    *Nutrition training, consultation and support
    *A focus on prevention and management of both medical and psychiatric illness
    *Ongoing consultation and collaboration between primary care and behavioral health staff
