The Care Coordination Program assists children from birth to age 18 who have multiple needs due to emotional or behavioral problems.
Program staff partner with families to encourage collaboration between service providers to meet the changing needs of the families.
The Care Coordinator partners with the family to:
*Identify the child’s strengths and needs
*Create a support team
*Develop a Plan of Care that addresses individual and family needs
*Review the Plan of Care each month to help the family work toward their goals and monitor progress
*Locate and coordinate services and supports in their community
*Advocate for services
